Who this assessment is for
This evaluation is built for situations where someone with authority needs evidence of how your condition limits functioning.

- You are appealing a disability decision and need functional documentation.
- You need evidence of how a condition limits daily activities, not only a diagnosis.

- Your claim file is missing an assessment of real-world functioning.
02 · What the assessment covers
What this assessment covers
The evaluation is anchored to the WHO Disability Assessment Schedule (WHODAS 2.0) framework, alongside symptom severity measures.
- Functional domains: self-care, mobility, cognition, getting along, life activities and participation.
- Symptom severity through PHQ-9 and GAD-7.
- Clinical and treatment history, plus review of records you provide.
- How limitations show up in daily routines and at work.
- Validated measures interpreted by a licensed clinician, with limitations stated plainly.
Self-report screening tools inform a clinician’s judgment. They do not, on their own, establish a diagnosis or eligibility for any benefit.

06 · How to use your results
How to use your results
The report is supporting documentation. Outcomes are decided by the agency or organization that receives it.
- Submit it to a disability agency, insurer or your attorney as supporting documentation.
- Use it alongside your own medical records and treating-provider statements.
- Note that the decision-maker — not this evaluation — determines what evidence it accepts and weighs.
- We never guarantee approval for any disability determination.
Honest scope. This evaluation is clinical documentation. Whether it is accepted is decided by the organization that receives it — not by us, and not by a score.
07 · Questions
Frequently asked questions
Will this get my disability claim approved?
An evaluation provides an informed assessment but cannot ensure a particular outcome. The agency or insurer decides, and it weighs all evidence. A functional report can strengthen a file by documenting impact directly.
Do you complete the agency's forms?
Your report is an independent clinical document. Your clinician will discuss at intake whether your case warrants any specific form or addendum.
How much does it cost?
The standard price is $695, with a normal range of $595–$795 depending on complexity. You are told before any additional charge applies.
How long does the process take?
The interview is 90 minutes. Records review and report-writing follow, and your clinician confirms the timeline at intake.
Can it be used for an accommodation request?
The functional analysis may be relevant to accommodation documentation. The employer decides under applicable law.
Who owns the report?
You do. We release it only to you or to a party you authorizze in writing.
