01 · Who this is for

Who this assessment is for

This is a shorter, occupation-focused evaluation for situations where the question is about work.

  • Your employer or HR has asked for documentation related to a condition.
  • You are preparing a reasonable-accommodation request.
  • You are returning to work after a leave and need documentation.
  • Symptoms affect attendance, focus, communication or stamina at work.
  • You want a clear picture of work-related functional impact, not a general assessment.
  • You need something you can share with your treating provider.

02 · What the assessment covers

What this assessment covers

The evaluation focuses on occupational functioning and the supports that might reasonably be considered.

  • How symptoms affect specific work tasks and demands.
  • Occupational and daily functioning through WHODAS 2.0.
  • Symptom severity through PHQ-9 and GAD-7.
  • Potential accommodations framed as options for consideration — not prescriptions.
  • Job-role context you choose to provide, including your essential duties.

Self-report screening tools inform a clinician’s judgment. They do not, on their own, establish a diagnosis or eligibility for any benefit.

03 · What to expect

What to expect

  • Time: a 75-minute video interview plus questionnaires completed in advance.
  • Format: fully online from a private space.
  • Clinician: conducted by a clinician licensed in your state.
  • Privacy: your information stays in your secure account.
  • Requirements: a short description of your job and duties helps the report be specific.

05 · What you receive

What you receive

Soft daylight across a plain plaster wall with the shadow of a plant

A signed, individualized 6–10 page report documenting work-related functional impact.

  • Clinical history and symptom severity.
  • A functional analysis of work-related limitations.
  • Potential supports and accommodations for consideration.
  • Clear boundaries on what the document does and does not decide.
  • Delivered as a secure PDF after quality-assurance review.

Report turnaround is confirmed by your clinician at intake.

06 · How to use your results

How to use your results

The report informs a conversation. It does not decide the outcome.

  • Share it with your HR team or employer as part of an accommodation request.
  • Share it with your treating provider to support ongoing care.
  • Accommodation decisions are made by your employer under applicable law — we do not determine them.
  • We never guarantee an accommodation, a job outcome or a return-to-work approval.

Honest scope. This evaluation is clinical documentation. Whether it is accepted is decided by the organization that receives it — not by us, and not by a score.

07 · Questions

Frequently asked questions

Will you tell my employer what to do?

No. The report documents functional impact and lists supports for consideration. Your employer decides what it will provide.

Is this confidential from my employer?

Yes. Nothing is sent to your employer unless you choose to share it.

How much does it cost?

The standard price is $550, with a normal range of $450–$650 depending on complexity.

How is this different from the full functional evaluation?

This one is shorter and occupational in focus. The Adult Psychological & Functional Evaluation is broader and produces a longer report for disability and legal contexts.

Do I need a diagnosis to have an accommodation request?

Accommodation processes and requirements vary by jurisdiction. This evaluation documents functional impact; it does not provide legal advice.

How long does it take?

The interview is 75 minutes. Your clinician confirms the report timeline at intake.
